NEW GUIDELINE AND PROCEDURES

The health and safety for our patients, families, and team members remains our number one priority. With that being said, there are some new changes to our check-in/check-out procedures for our patients. We have prepared a video for each office to show what that entails. One thing that we did not mention in our videos is, with the new protective coverings our staff will be wearing the office temperatures may be colder then before. We suggest patients may want to bring a jacket or sweater to their appointments.

INFORMED CONSENT AND COVID-19 PATIENT SCREENING

At each patient appointment the parents/guardians are given 2 forms to complete prior the patient seeing the dentist. The first form is getting the parents/guardians signature to accept the risk and consent to treatment for their child to be seen. The second form is a Covid-19 Patient Screening. In order to reduce the risk of spread of COVID-19, we have to ask you a number of screening questions. 

If the parent or guardian is unable to bring the patient to their appointment they must print off these forms and send them with the patient to their appointment.
